Health Form Sample- It is the responsibility of the Club Director to have readily available the health information of the Pathfinder and Staff under their care. Each conference has their own way of collecting this data. We are putting here an example of the form you may want to use. In the event of a health related emergency the pathfinder club director will need to have this information readily available.
Letter of Excused Absence from School
Liability Waiver - All Adults and Pathfinders registered to attend the camporee must complete and sign the waiver form and submit it to their local Club director.
Club directors will bring all completed and signed waivers to Summit Bechtel Reserve and will present them at the time of check-in.
No Adult or Pathfinder will be permitted on the campgrounds without a signed waiver.
